Care and treatment is underpinned by a multi disciplinary team approach that focuses on evidence based practice within each discipline. Assessment is a continuum from admission through to discharge.
Some of the basic assessments that are currently used to formulate patients individual treatment plans are outlined below:

Battery of psychological assessments including assessment of personality

Social worker home and family assessments

Psychiatric assessments

Health of the Nation Outcome Scale

Patients are encouraged to take an active responsibility for their treatment and rehabilitation programme, and in the day to day life of the hospital community

Structure provided by comprehensive Therapeutic Working Day programme of individual and group work, OT, leisure and sporting activities

The MDT works collaboratively with patients care pathways and Care Programme Approach (CPA) framework
- The MDT also develop individual Psychological Treatment Pathways for each patient which includes a number of therapies. This has been created using evidence based practice, for more information click here
