Referrals can be made in writing, by telephone or via e-mail by any healthcare professional.

Support from the relevant funding team would be preferable although it is not necessary as we can contact them on your behalf, if requested.

Once a referral has been made the Referral & Admissions Co-ordinator will arrange a pre-admission assessment to take place.
